Licensed Practical Nurse (LPN)

Position Overview

Oak Bay HR is seeking a compassionate and professional Licensed Practical Nurse (LPN) to deliver senior‑focused care in a supportive, person-centered environment. In this role, you’ll provide direct nursing care using individualized care plans, while serving as a mentor and leader within the care team—aligning closely with your organization's values and culture.

Key Responsibilities

• Clinical Care & Assessments

Perform nursing assessments, administer medications, wound care, and record vital signs including temperature, pulse, blood pressure, and respiration. Collaborate on developing and evaluating resident-specific care plans.

• Care Collaboration & Communication

Collaborate with Registered Nurses and the broader care team to ensure cohesive delivery of care. Participate in care conferences and shift reports to ensure transparency and continuity.

• Leadership & Mentorship

Supervise, guide, and support Continuing Care Assistants (CCAs) and Personal Support Workers (PSWs) to promote safe and compassionate care practices.

• Documentation & Safety

Maintain accurate and timely medical records and documentation of care. Ensure care delivery aligns with clients safety and quality standards.

Qualifications & Requirements

• Active LPN registration with the relevant provincial regulatory body.

• Flexibility to work varying shifts, including days, evenings, weekends, and holidays.

• CPR & First Aid certification (or BLS) and a clear criminal record with vulnerable sector screening.

• Previous experience in long-term care or senior-focused healthcare is an asset. Specialized training (e.g., Alzheimer's/dementia care) is a plus.
